Our Chronic Disease Management (CDM) Clinic provides care and ongoing support for patients with long-term health conditions. Our General Practitioners and Registered Nurses work together to develop personalised care plans tailored to your needs.

Who can access chronic disease management?
The CDM Care Plan system is available to patients with chronic conditions who meet the eligibility criteria. A chronic disease is a health condition that has lasted, or is expected to last, six months or longer.
